What is the forms and surveys module?
The tool the school uses to build its own forms: a one‑on‑one talk, parent contact, an assessment, an incident, a satisfaction survey, a staff questionnaire and more. The form is sent to the audience you chose, the answers are summarized on a dashboard, and every completed form is saved in the student file or the staff file.
How do you build a new form?
In three steps. Choose whether to create it with AI, from a short description in your own words, or to build it by hand. Give the form a name and a description and add questions. Finally set the audience, the permissions and the deadline.
Which field types can a form have?
12 types: short text, long text, date, dropdown, single choice, multiple choice, a 1–10 scale, star rating, file upload, notification, student picker and staff picker. Any question can be marked as required.
Who can fill in a form?
There are three options: the student fills it in themselves, a staff member fills it in about a student, or a staff member fills in a form meant for staff. You decide this when you build the form.
Where is a completed form saved?
A form filled in about a student, or by the student themselves, is saved in the Records tab of the student file. A form meant for staff is saved in the staff file of the person who filled it in. Nothing needs to be filed by hand.
Can a form come back every week?
Yes. In the recurrence settings you choose the days of the week, how many weeks apart and an end date, and the form is due again each time.
What happens when a form is submitted after the deadline?
You decide. You can allow late submissions, which are marked as late, or block submissions after the deadline. When the deadline is getting close, the system sends a notification.
Who sees the form and the answers?
Only people with permission. For every form you set separately who on the staff may view it and who may edit it: all staff, or selected staff members.
How do you see the results?
The forms dashboard shows how many forms were filled in against the target and which forms have the highest fill rate. Every form has a responses table and a dashboard showing how the answers to each question split. Responses can be exported to Excel or PDF.
Can a form be changed after it was sent?
Yes. An existing form can be edited: add questions, change their order and update the audience, the permissions and the deadline. You can also deactivate a form, and it will not be shown to users until you activate it again.
We have forms on paper and in files. What do we do with them?
Build them in the system once. Describe the existing form to the AI, or add the questions by hand, and from then on people fill it in on a phone or a computer.
Where is the data stored?
On servers in Israel, on AWS. Smart Campus is certified to ISO 27001 and ISO 27799.
